Pediatric Ophthalmology Consultants

Payment Policy

For the convenience of our patients we accept Visa, MasterCard and cash (no checks accepted). We are a preferred provider for main insurance companies. At the time of scheduling your appointment a determination will be made as to whether you will need to pay the entire cost of the visit. Many insurance plans pay for medical eye problems and not for routine eye exams for glasses or contacts. Many insurance plans have a deductible. Some require a referral from your primary physician. For these reasons we will determine whether your insurance will cover the cost of the office visit at the time of the appointment. If your visit is covered by your health plan, we will be happy to file claims for you. We do not bill for office visits or co-pay amounts. We expect full payment before your eye examination. If you or your child requires surgery, we will be happy to schedule a surgical appointment, we generally have opening for surgery within 2 months. We will call your insurance company to determine your coverage and to obtain pre-certification. If your insurance company will not cover the cost of the surgery, or will only cover a small portion of the cost, prepayment will be requested.

Thank you for choosing the practice of Dr. Roberto Warman & Staff, we look forward to seeing you soon.
